I believe that if they leave the cave, they no longer have immortality, whether or not the grail is there. Obviously the knights brother didn't have immortality, and he walked out of the cave 150 years later. Also, I think the grail is still there, and the night is still alive, guarding it. Perhaps Hannibal King is correct in saying that you have to drink from it every day to maintain your immortality.
