That is real; but not chronic.
You can notice a limp in the Fugitive for one.

Basically, he tries to do as much of the stunts as he feels safe doing.
Sometimes he has gotten hurt and his condition reflects it in some of
his work.

Some of his best performances were:
dragging behind the truck in raiders and
jumping onto the moving tank in Last Crusade.
