In the case of "Star Wars", Lucas did add "Episode IV: A New Hope" to the title when the movie became successful so that it would match with other Episodes, which makes sense (I didn't see it until LONG after it was out so I've always known it as "A New Hope" anyway). Presumably, he thought about putting it in when the movie first came out, but since he didn't know if he'd be able to make the others, he left it out.

I wasn't aware "Star Trek II: The Wrath Of Khan", didn't have the "II" when it came out, but when I think I about it, I can see why since the movie had very little in common with the original film (wise move. That first one is the worst of the series). When was the "II" added, by the way? Was it close to the release of "Star Trek III: The Search For Spock" (if so, I'd say that would be the reason for the change)?
