In the game Infernal Machine, you get a chance to actually see Satipo, and the way the spikes are lined up is that there is about 5 or more of them, each placed about 5 inches apart.

Now considering that the top spike went right through his throat, there is a good chance that one of the spikes did go through "there". No, it's not Ouch, it's DAAAAAAAAAAMMMN!
