I agree with Michaelson and recommend you add a little (1") to the shoulder measurement! You should measure from the point where the sleeve meets the shoulder, following the natural curve across the shoulder to the corresponding point. Measure the sleeve lenght from the exact same spot, bend your arm slightly and measure around the elbow!
Should you have a jacket that fits you just right, you may want to take the exact measurements from that!

The leather facings on the collar and along the zipper did NOT cost extra. You may want to ask for a zippered interior pocket (no extra cost).

Prepare yourself for a long wait. But it's REALLY WORTH IT!
