Bullet wounds are achieved with devices called squibs. they're small blood packets, detonated by remote via a wire hidden under the clothing. They're not powerful eough to blow through the garment, so the fabric is scored where the hit will take place. It's difficult to do, as the gunshot and the detonation have to be PERFECTLY synchronized. If you're interested check an issue of either Cinefex or Fangoria.
