The story as I know it, the producers were trying to cut corners. They let the union prop master go. A cheaper non-union guy was left in charge. Also It was my understanding that the fake-dummy rounds had lead bullets. When the dummy was replaced with a blank, the lead bullet from the dummy had slipped out of the cartridge into in the barrel. So now you have a blank in the chamber and a bullet in the barrel. When fired the bullet was projected at a low velocity. The lower the velocity the greater the penetration. Also the bullet would have been tumbling. A real tragedy.
