Temple of Doom Question

I have always wondered why Mola Ram tore out the first guys heart but then didnt tear out Willy's. Was this just because the scriptwriter decided she needed a heart in the rest of the movie? Mola Ram didnt want to damage her breasts? The first one was an illusion and he actually didnt tear the heart out? What do u guys think?
