In Wolfram von Eschenbach's "Parsifal", the grail is not a cup, but a stone. Wolfram, who was a German knight and a minstrel (or minnesanger), claimed he obtained the info on the grail from reliable sources.
The first to 'invent' the grail and include it in his works was, I believe, Chretien de Troyes. It was he who described it as a plate.

You'll find more info in Jessie Weston's book on the grail (the title escapes me now). And in the original chansons de geste ;-)
