That the flames are still burning is clearly a mistake, since the kind of fireball that moved through the catacombs MUST have consumed most of the oxygen (at least temporarily). In any case, the smoke and fumes, and the resulting lack of air should have finished off Indy and Elsa. Flames just look so pretty on the screen...
