Seeing as how Indy's friend wasn't moving at the time or filled with an adreniline rush, where his mind would be destracted, I'd have to say he certainly should have realized (felt) he'd been shot. After all, he was just passively standing there. Not like in Witness where Harrison was shot in the heat of a gun battle and didn't realize until afterwards he'd been shot. In this case, the waiter definately would've been aware. You don't get whacked with 100s of pounds of energy (foot pounds in gun lingo) and not know it.

Still, I received a grevioius injury once, and didn't realize it right away. Those things happen so fast, it takes awhile for the body to register it's been injured.
