um, either med school or a Ph.D. program is the usual way... But there are far too many academics who have never published more than their name in a phone directory. I suppose we could bandy about different titles used in different cultures ("Herr Doktor") or by different, how shall we say, individuals ("Doctor Hunter S. Thompson") that require an altogether different protocol... But it's up to you. As an old prof of mine used to say, "I don't do surgery; don't call me doctor. Just call me 'sir'". Quite a jovial fellow, that man. There's really nothing to prevent anyone from calling themselves whatever they want; this may or may not fly in their respective neighborhood. In this happy little forum the situation is much more relaxed: "What, We Worry?"
